*Youth age 6-21 are only $10 per month with any parent or legal guardian that joins. Family membership includes two adults and two children, additional children can be added to your subscription for an additional $10 per month. please note, To enroll in membership as a family all members must reside in the same household. Independent adults ages 18-21 pay membership pricing of $39 per month based on their age bracket.

Complete physical exams and routine checkups for patients of all ages. Annual Physical Exams (all ages, male and female) Includes sports physicals, school physicals, camp physicals, etc.


Extended office visits (30-60-90 minutes) with no co-pays.


All membership house calls and telemedicine visits (webcam, email, phone) are provided to you without any additional fees.

PAYMENT OPTIONS AND DISCOUNTS





Payment Options
Option 1: Memberships can be paid in 12 monthly installments as a recurring credit or debit card payment due by the 15th of each month.
Option 2: A discount of 15% will be applied to the annual membership fee if paid in full by credit/debit card, check, or cash at the time of signing the agreement and on the yearly anniversary thereof (can be combined with the couples and occupational discounts below).




Couples Discount

Empty Nesters? No Children? No Problem! Couples (2 Adults) who subscribe receive a 15% discount off of their combined monthly subscription.



Occupational Discount

As a thank you to those who work tirelessly to keep our community safe, we proudly offer a 10% discount to first responders and police officers. We also extend this discount to active/retired service members and teachers.
